Hi, this is my first post on SitePoint!
I usually use a form builder to create the basic forms I need, but I am needing to create something that exceeds my form builders capabilities.
I am needing a form where someone can enter a few fields of info ie: name/dob/expiration date, and the form will confirm that the person is a current patient. Basically, once you answer the three fields correctly it will take you to a Success page. The list of patients and their info can be on an excel sheet somewhere. Then all I have to do is update the excel sheet with new patients and the new patients info will create a success page when entered in the form.
