I downloaded the first chapters and it looked fine. My question is does the book go on to actually build the forms in question? - does it include the html, CSS, and I guess links to any graphics in use.
We're thinking of expanding to another web person and kind of feel we would like to give a College kid a go. Kevin Yank's box is fine for php but am after something to show form design and CSS
Not so keen on a book that simply waffles on about the design process as we have our own development methodology.

It goes into a lot of detail about the HTML, CSS and also JS involved. Not sure if you can download any images, but it shows how to create them in Photoshop. (I don't think there are many, though. It's mainly CSS-based styling.)
