Look in PHPMYAdmin.
Create a table with a date or time field.
Use PMA to insert a row, next to the date field you will see a date picker.
Pretty nifty - but that should get you going.

I found this site: http://www.mattkruse.com/javascript/calendarpopup/ very useful. Pretty easy to get going. It is not exactly the same as on the link you provided, but there are a number of different calendar options and you can use javascript to adjust it as needed.
