Execute php in pages?

in one of my wordpress pages (contact) I have some php code,

so I installed this plugin

I activate it, and this is the result,

Did I do something wrong?

tried it again at

no dice

I created a page which uses exec-php to execute some php code.
My page seems to not fully load

And I believe the culprit is
this


require_once("./include/fgcontactform.php");
[\\php]
How do I even know if that path is even correct?

I have an include folder in my sites root.
Heres the script
 (I placed a 1 there to see if it even works)

I guess the php is working, think the problem is require_once()
