Form not displaing on iPhone but does on desktop and other mobiles

Hi,

I have a booking form that displays fine on a desktop and other mobile devices, except on an iPhone 5. Does anyone know what could cause this? I know javascript is working on the iPhone, but not sure why the form isn’t displaying.
