How to call another form after submitting one form

All

I have form which is having one field called student id. When user enters the student Id and clicks on submit button It is saving to the table. Now I want to display another form as soon as user clicks on the submit button which should show the student I d and another empty field like student name

Please help me out

how does the user of the first form know the id of the student?
