Looking for a quick solution for registration of students for different programs

I’m looking for a web service that provides online registration for different programs. I need to be able to set the fields I need like (first, last, phone number etc).

I need to be able to insert the programs names there are about 20 of them. Each registrant will fill the form and select the program he desire.

that there should be a button for paypal payment.

Is there a web service that can provide me with this kind of solution?

Use WordPress and s2Member plugin.

this is supposed to be a one time thing, so setting up wordpress and using a plugin seems a bit an over reach now.

Thank for the idea anyway, it’s looks awesome.

Any other methods? maybe some kind of a web service that offers this kind of form? like eventbrite does for events?
