Making fill-up form for Begginers

I want to make my web page like this, that has a fill-able form.

Hey @Alvie — What’s your experience with HTML, form processing and all that? In terms of the HTML involved, you can just look under the hood to see how that’s done via Inspect Element. In terms of the backend code to process it, that’s a different matter.
