Recommend an online Form tool

I’m trying to customize the form plugin Contact-7 in Wordpress with something like Uni-form (http://sprawsm.com/uni-form/) or jqTransform (http://www.dfc-e.com/metiers/multimedia/opensource/jqtransform/)?. But it’s not working.

Instead I’d like to use a separate form tool that isn’t a wordpress plugin. Any recommendations? There’s formstack?

Any other suggestions?

There will not doubt be a style sheet through which you can change the look of it. Are you talking about the look, or the functionality?
