Simple Survey Script

Hi all,

I’m looking for a simple survey script, I want to be able to ask users a few questions on what they think of our services, ask them for their name and email, thank them and have the results emailed to me for record keeping.

Any ideas?

Was look into something more… already made, not having to re-create the wheel.

create a form, create a database table have the information inserted into the table. walla.
